Acute coronary syndrome and platypnoea-orthodeoxia with thoracic and interauricular septal aneurysms.
OBJECTIVE Platypnea-orthodeoxia is a rare syndrome characterized by dyspnea induced by the upright position and relieved by supine position and an arterial deoxygenation increased by the upright position which improves during recumbency. Several anatomical factors that can alter the atrial anatomy and facilitate shunting through an interatrial defect have been related to this syndrome. In many ...
متن کاملCARDIOVASCULAR MEDICINE Hypoxaemia associated with an enlarged aortic root: a new syndrome?
Objective: To assess the mechanisms through which an enlarged aortic root may facilitate right to left shunting through a patent foramen ovale. Patients: 19 patients with the platypnoea-orthodeoxia syndrome (POS) were compared with 30 control patients without platypnoea. Interventions: Multiplane transoesophageal echocardiography. Main outcome measures: The aortic root diameter, atrial septal d...

A patient with a large hydatid cyst of the liver developed a positionally symptomatic right to left shunting across a patent foramen ovale with both platypnoea and orthodeoxia, despite normal pulmonary arterial pressures and normal pulmonary function tests. When the patient was in the supine position the calculated right to left shunt was 15.1% and 29.5% when seated. The shunt was attributed to...
متن کاملPlatypnea-orthodeoxia due to aortic elongation.
A67-year-old woman admitted to the hospital with syncope was found to have platypnea-orthodeoxia caused by a dilated aortic root. A transesophageal echocardiogram was taken in the supine and sitting positions (Figure). The images taken in the supine position showed the foramen ovale was closed. The images taken in the sitting position show the foramen ovale was wide open, with a massive right-t...
